Koronare Herzkrankheit (KHK)

Das Krankenhaus Angermünde bietet Patienten mit koronarer Herzkrankheit die Teilnahme an einem Disease-Management-Programm (DMP) an. Die Therapieziele sind das Aufhalten des Fortschreitens der Erkrankung sowie die Steigerung der Lebensqualität. Die therapeutischen Maßnahmen umfassen neben der medikamentösen/interventionellen Therapie auch Ernährungs- und Raucherberatung, körperliche Aktivität, psychosomatische und -soziale Betreuung sowie die fortführende Rehabilitation.
