Koronare Herzkrankheit (KHK)
Bei den sog. „Disease-Management-Programmen“ (DMP) handelt es sich um spezielle strukturierte Programme für chronische Krankheiten. Diese zielen darauf, die Versorgung und Betreuung chronisch Erkrankter zu verbessern. Der Universitätsmedizin Rostock ist im Falle chronischer Erkrankungen ein gut abgestimmtes und kontinuierliches Vorgehen bei der gesundheitlichen Versorgung sehr wichtig, um etwa das Risiko für Komplikationen oder Folgeschäden reduzieren oder vermeiden zu können.
