Diabetes mellitus Typ 2
DMP sind Behandlungsprogramme für chronische Erkrankungen wie Diabetes mellitus, für die bestimmte Behandlungsabläufe festgelegt wurden. Diese sogenannten strukturierten Behandlungsprogramme haben das Ziel, Patienten mit chronischen Erkrankungen besser als bisher zu versorgen. Durch die Teilnahme von niedergelassenen Ärzten und Krankenhäusern soll erreicht werden, dass die Patienten in einer lückenlosen Behandlungskette gleichmäßig gut versorgt und ihre Behandlungen besser koordiniert werden.
