Das Dietrich-Bonhoeffer-Klinikum nimmt seit 2004 am DMP Diabetes mellitus Typ 2 teil. Ziel ist eine sektorenübergreifende, indikationsgesteuerte und systematische Behandlung. Durch die Anwendung evidenzbasierter Leitlinien, sektorenübergreifender Behandlung und individualisierten Therapien soll die Lebenserwartung erhöht und die Lebensqualität erhalten bzw. verbessert werden. Diabetes mellitus Typ 2 zählt zu den häufigsten chronischen Erkrankungen im mittleren und höheren Lebensalter.
